City Center Workforce Development
Ready4Work was developed in 2002 by the Department of Labor’s Center for Faith Based Initiatives as the pilot site of the Ready4Work initiative. Jacksonville Ready4Work brings together a collaboration of existing programs that together create a web of services for returnees. Two faith-based organizations have thus far combined efforts to provide leadership for this program: Operation New Hope and City Center Workforce Development. Operation New Hope was formed in January 1999 to provide affordable housing in the historic Springfield District of downtown Jacksonville and the surrounding urban core, and today provides job training and job placement in the construction field for returnees. City Center Workforce Development recruits participants into the program from prison facilities, partners with other organizations to recruit mentors from the faith community and supplies case management to participants and mentors. Gateway provides a full continuum of care that delivers effective treatment and habilitation services to individuals suffering from alcoholism, substance abuse and related mental health problems. Gateway provides comprehensive prevention/intervention treatment services. Prevention services for adults and adolescents include community education, child welfare/substance, abuse workshops, web-based educational services, student assistance programs, children of alcoholic groups and HIV/AIDS training. Intervention services for adults and adolescents begin with a professional assessment to determine the most effective and least restrictive treatment available to each person coming to Gateway. Services include detoxification, crisis intervention, assessment, individual and group therapy, family therapy, relapse prevention and aftercare. Intervention programs include: The Juvenile Addiction Receiving Facilities service for adolescents and Detoxification Services for Adults providing twenty-four hour medical supervision in a secure environment; Residential Treatment; Outpatient Services; and Transitional Housing with Supportive Services. Services are based on the 12-step recovery model in partnership with best practices associated with co-occurring mental health and substance abuse problems. City Center Workforce Development
Ready4Work was developed in 2002 by the Department of Labor’s Center for Faith Based Initiatives as the pilot site of the Ready4Work initiative. Jacksonville Ready4Work brings together a collaboration of existing programs that together create a web of services for returnees. Two faith-based organizations have thus far combined efforts to provide leadership for this program: Operation New Hope and City Center Workforce Development. Operation New Hope was formed in January 1999 to provide affordable housing in the historic Springfield District of downtown Jacksonville and the surrounding urban core, and today provides job training and job placement in the construction field for returnees. City Center Workforce Development recruits participants into the program from prison facilities, partners with other organizations to recruit mentors from the faith community and supplies case management to participants and mentors. Gateway provides a full continuum of care that delivers effective treatment and habilitation services to individuals suffering from alcoholism, substance abuse and related mental health problems. Gateway provides comprehensive prevention/intervention treatment services. Prevention services for adults and adolescents include community education, child welfare/substance, abuse workshops, web-based educational services, student assistance programs, children of alcoholic groups and HIV/AIDS training. Intervention services for adults and adolescents begin with a professional assessment to determine the most effective and least restrictive treatment available to each person coming to Gateway. Services include detoxification, crisis intervention, assessment, individual and group therapy, family therapy, relapse prevention and aftercare. Intervention programs include: The Juvenile Addiction Receiving Facilities service for adolescents and Detoxification Services for Adults providing twenty-four hour medical supervision in a secure environment; Residential Treatment; Outpatient Services; and Transitional Housing with Supportive Services. Services are based on the 12-step recovery model in partnership with best practices associated with co-occurring mental health and substance abuse problems. Gateway provides specialized programs for the unique needs of women with children, high risk youth, children in foster care, people in recovery who are homeless, person at risk of contracting HIV/AIDS and adolescent involved in the juvenile justice system.

Provides health care and linkage to specialty medical care to homeless and uninsured poverty stricken persons living at or below 200% of the Federal Poverty Guidelines. Services are provided on-site at the Sulzbacher Center on a first come first served basis. Services include, but are not limited to the following: primary health, dental, and vision care, pediatric specialty care, asthma clinics, diabetes screening, TB testing, psychiatric clinics, HIV/AIDS testing and case management, minor surgical procedures, and pharmaceutical supplies. Patients are screened and triaged to other specialty care programs as needed and as services are available.
